Effective February 1, 2008, the physicians of Fremont Pediatric Medical Group (FPMG) no longer see patients in private practice, but continue to practice in the same office, now with Kaiser Permanente. The office at 43971 Boscell Road is now known as Kaiser Fremont West Medical Offices. The office accepts only Kaiser Permanente health insurance plans, but will also see patients who pay cash at the time of service.

Medical Records
Medical records were maintained at Boscell Road until November, 2009, to give families ample time to access their children's records, but all FPMG medical records are now stored off-site. If your child was a former patient of FPMG, and you wish to obtain a copy of his/her medical record (this includes requests for immunization records), you will need to call our records custodian, Underground Vaults and Storage, at 1-800-873-0906, option “0”. There is a charge for records copying and mailing. You will be asked for identifying information to verify that you are the legal guardian of the patient. After talking to Underground Vaults, you will be asked to send this form [RELEASE OF MEDICAL RECORDS] to them. Please do not fax this until you have placed your request over the phone—fax-only requests will not be processed. Please allow up to 7 business days for records to be copied and mailed to you.
If you need to contact one of the former FPMG physicians for a matter not involving medical records, they may be reached by mail at the address shown at the bottom of this webpage. Please always include your child’s full name, date of birth, and approximate year of last office visit to FPMG in any correspondence, as well as the best way to contact you. We are not able to email patients privately; please include your phone number.
